Everytime something is going great I always do something or say something to totoally mess it all up. I try so hard and then lose everything. I must be pretty dumb to think that I can ever get this right and be happy.

Getting it right every time, and being happy all the time, are both unrealistic expectations. We have to accept the fact that we are not perfect, and we are not expected to be perfect. We also have to accept that we will not be happy all the time. Nobody ever said that we would be.
